Application
The Downloadable Application is available in two versions:
View and Print version allows you to print the unfilled form and fill it out with a typewriter or by hand
Type On and Print version which allows you to fill out the form in Acrobat and then print out the final version.
Please note that you must print out at least two (2) copies of the GRADUATE SCHOOL APPLICATION (pages one and two), and at least one copy of the APPLICATION FEE FORM (page three).
You will need Adobe Acrobat Reader to access the downloadable application.
If you do not currently have Adobe Acrobat Reader, please click here to download the free version of Acrobat Reader for your computer.
Mail one copy of the official GRADUATE SCHOOL APPLICATION along with your payment of the non-refundable application fee to Graduate Enrollment Services at the address indicated below.
